Terms of use of gift certificates from "Leleka"

Dear certificate holder!

We congratulate you, because in your hands you are holding a gift certificate, with which you can receive high-quality medical care in the departments of the modern and innovative Leleka Medical Center at the addresses:

• Lesya Ukrainka Boulevard, 23-B, 2nd floor, Kyiv.

There are several conditions for using gift certificates from Leleka 

If you have a gift certificate for a specific service, service package or visit to the Leleka  restaurant, the following conditions apply:

1. The certificate has a term of validity that is specified in the certificate, or is valid for one year from the date of delivery, and cannot be extended. If you have not used the gift, it will be canceled.
2. Can be used only for the service specified in the certificate.
3. You have the right to transfer it to a third party.
4. It is not possible to receive cash equivalent to the service specified in the certificate.
5. The Gift Certificate is non-refundable.

If you have a gift certificate for Leleka medical services with the specified monetary value, the following conditions apply:

1. You have the right to receive medical care for an amount greater than that specified in the gift certificate by paying extra for it.
2. If you receive medical assistance for a lower amount, the monetary difference is not refunded.
3. The certificate can be used only during one visit to the medical center.
4. If the amount of the service is less than the nominal value of the "Gift Certificate", then the difference between the price of the service and the nominal value of the "Gift Certificate" is not compensated, is not carried over to subsequent visits, and is not subject to payment in any form.
5. The certificate has a term of validity that is specified in the certificate, or is valid for one year from the date of delivery, and cannot be extended. If you have not used the gift, it will be canceled.
6. It is not possible to receive cash in the equivalent specified in the certificate.
7. The specified conditions apply, unless otherwise stated on the gift certificate.
